FreeMed
-- According to their Web site, "FreeMED is open source practice management and electronic and computer records system. It is Episode of Care based, differing from conventional billing records. It allows the tracking of medical data, in detail, with preservation not just of the diagnosis but the reasons for medical encounters."
OpenHRE.org
-- OpenHRE stands for Open Source Health Records Exchange. According to its Web site, the mission of OpenHRE.org is "to foster development, distribution and support of Master Patient Index and Health Record Exchange systems and components held as Free/Open Source Software; to build a community to this aim; and to realize this goal via a self-sustaining business model and open collaboration among all stakeholders."

tkFP Project
-- According to their Web site, tkFP is "an electronic medical record information system suitable for a solo or small group physician offices for storing clinical information on patients and other functions needed to run a small medical practice. It’s really a collection of the best free stuff we could find on the internet combined with some of our own code to get the components to work together. tkFP is currently running and in daily use in two separate dual physician family practice medical offices with over 5700 registered patients, one in California and one in Iowa."
